What do "medium-firm" and "firm" actually mean?
There is no industry standard for the 1 to 10 firmness scale used on mattress product pages. Each brand assigns its own number, so a 7 from one company can feel like a 6 or an 8 from another. As a rough guide, medium-firm is usually described around 6 to 7 on brand scales and firm around 7.5 and up, but those are conventions, not measurements. The label describes how far the surface gives before the support underneath takes over, not how supportive the bed is. Research uses a different scale. The Lancet trial reported mattress hardness on a scale where lower numbers are firmer: its firm mattress scored 2.3 and its medium-firm mattress 5.6. That scale does not convert cleanly to any brand's 1 to 10, so be wary of any page claiming science proves a specific number such as 6.5.

What are the two most common firmness mistakes?
Buying firm to fix back pain. The trial evidence points the other way for chronic low back pain. That does not mean firm is wrong for everyone; it means "firmer is healthier" is not what the research found.

How do you decide without guessing?

- Try the hand test. Lie on your back and slide a hand under your lower back. As a rule of thumb, a hand that slides in with a little resistance suggests a neutral curve; a large gap suggests the bed is too firm for you, and no room at all suggests too soft. It is a home check, not a clinical measurement.
